Medication Delivery

Enjoy the convenience of having your medications delivered to your doorstep or opt for self-collection at CCMK Pharmacy.
This service is available for patients with a valid prescription from KMH who wish to purchase the remaining uncollected prescribed medicine as stipulated in the Partial Collection of Medicines memo and have no change in dosage since the last collection of medicine.

Know Your Medicine

Medicines play an important role in helping you to manage your illness, especially if you have a long term medical condition. CCMK Pharmacy is committed to providing you with the necessary medicines and advice to use your medicines safely.

Medicines for High Blood Pressure and Diabetes (Type 2)

There are many types of medicines to help control your blood pressure and/or diabetes. It may be necessary for you to take more than one medicine to keep your condition under control. It is important to take your medicine as prescribed by your doctor or pharmacist. Otherwise, the medicine may not work, cause undesired side effects, or result in harm to your body.

Self-Care and Self-Medication

Self-care is broad concept which includes hygiene, nutrition, lifestyle and self- medication.
Not all medical conditions may be suitable for self-medication. Self-medication should only be practised for minor illnesses such as acne, cough and cold, minor cuts and wounds and fungal infections of the skin. However, if symptoms persist after one to two weeks or worsen, stop self-medication and consult a doctor.
You should not self-medicate for chronic conditions such as high blood pressure and asthma, bacterial infections and in infants and the elderly.

Prescription Filling

Patients seen at our KMH clinics will no longer receive paper prescription by default, except for purchasing of medications that is not within the stock list of KMH Clinics. Medication orders prescribed by the doctors are electronically sent and stored in the pharmacy system. If required, a medication list can be provided to the patient, in place of the prescription, for reference.

Use the OneCCMK App for Medication Top-ups & Refills

The OneCCMK app allows patients to order their prescribed medication top-ups and refills through the mobile application.
For your safety and to reduce your wait time in the polyclinic, we strongly encourage all patients to use our medication delivery* service and have your medications delivered to your preferred location. You may place your order request using your medication list as reference through the OneCCMK app.

Knowing My Medication

Knowing your medication can help you practice safe and effective self-care. This can either be done by reading the labels and patient information leaflet attached, or by asking a healthcare professional.
Conversely, lack of accurate information may lead to unwanted interactions between medicines, medicines and supplements or medicines and food. It may also lead to over or under dosage, and risk of adverse drug reactions.
